The Advanced Bobath Course in Barcelona - Day 2
Assessment, clinical reasoning and specificity of handling –
that’s the topic of the Day 2 in the Advanced Bobath Course. I really have to
say that it is so amazing to see Lynne working with the patients. How she
really handles them, how she get them to do things and teaches them to use
their body more effectively and so on. Today her treatment demonstration took 2
hours but she got excellent outcomes and the patient was very satisfied.
Some questions for today:
-
is there something wrong in the alignment?
-
Is some sequence missing from the movement
pattern?
-
Is there stability enough to be mobile or
mobility enough to reach stability?
We were practicing with each other for example how to get
from standing to sitting and to lift the leg to the bed and the other one next
to it. In fullsitting position we then did some weightshifting and core
stability trainings and gradually landed to supine. It sounds easy? My partner
was a bit challenge for me J.
Luckily I got some help from Lynne and we managed to get her in sitting
position. It also showed that most of us has some differentiated movement
patterns because of some pain or trauma. One case was one of the
students who had problems to move weight to right leg in standing. The same
problem was also in sitting and supine. The whole kinetic chain was changed
all the way from the foot to the head. There
were lots of compensation from the body and Lynne showed us how the patterns
work!
Very nice day with full of Hands On –work! In the afternoon
it got sweaty when we had the own patients. In the end of the session I had a
chance to co-work with Monica (Bobath instructor and the assistant of the
course). She showed me hand placements and helped me with the patient to get
him in one-leg stance. It was very hard because the postural control and core
stability were not so good. Anyway it was successful try and lasted few
seconds. The patient was satisfied and did a great job! Muy bien!
Something to consider…
It is quite usual that more affected side is going back from
the hip when you should take a step forward. We were doing some work with
sacrum and ileum today. Ileum should get lower in relation to sacrum to get
more range of motion to step forward. Lynne demonstrated this very well with
the patient and got very good effort.
The other thing was when practicing bridging or unilateral
bridging. You need to get the core activated too when working with hip
extension. When facilitating from the proximal part of the hamstrings you can
give the information to the hip that the patient can find it and activate the
core also. We should avoid to stimulate the extension of the trunk because it’s
not very useful in general movements.
It is so calming to sit on the beach and nothing but the ocean ahead..
